When does an aortic aneurysm need surgery?
Generally: ascending aorta >5.5cm (or >5cm with risk factors), descending thoracic >6cm, abdominal >5.5cm. Patients with connective tissue disorders (Marfan) need earlier intervention. Rapid growth (>0.5cm/year) is also an indication.
What is acute aortic dissection?
A tear in the inner layer of the aorta allowing blood to flow between layers. Type A (involving ascending) requires emergency surgery. Type B (descending only) can often be managed with TEVAR or medications. This is a surgical emergency with high mortality if untreated.
What is frozen elephant trunk?
A hybrid procedure combining open surgical arch replacement with deployment of a stented graft into the descending aorta. Allows single-stage treatment of extensive aortic disease involving arch and descending segments.
Can TEVAR be used for all thoracic aneurysms?
No. TEVAR requires suitable landing zones (at least 2cm of healthy aorta above and below). Arch aneurysms often require open surgery or hybrid procedures. Proximal descending aneurysms are ideal for TEVAR.
